Mach3 does not do kenematics so you do NOT have world point jogging of the B axis. As to the setup to generate Gcode that would be UP TO the CAM program You would have to have a 4/5 axis CAM and setup the axis offset there so it KNOWS how to generate the proper geometry for the part and tool offsetting.
